KKM form? MMC form? SPA? eHO? Guideline for newly graduated doctors.

Salam.

I received a few messages from friends from other universities asking me about what to do with KKM form, MMC form & eHO. I was one of those who helped my UM batchmates to settle this stuff but if you need further clarification, please kindly contact KKM yourself. This is just a guideline that we, UM graduates did.

I understand that some of the universities already sent KKM form before their final exam which is the conventional way. However this year our government has changed to online system which is known as eHousemen. I am not sure if you need to submit the new KKM form or not because we didn’t use the conventional way. We did not do anything before final exam. We just went to SPA interview. That’s it.

So this was what we did after we passed our exam.

1. Register with Malaysian Medical Council (MMC)


*All photostated documents need to be verified.
* You have to submit your ORIGINAL Deans letter.
* Only 1 passport photo.
* RM20 wang pos. If you buy from post office you will be charged RM1 but if you buy from bank you will be charged RM6.
* Please submit to MMC in BUNDLE. The process will be faster if you send in bundle.

2. Submit KKM form

We received the new Borang Maklumat Diri from KKM staff who emailed it to me. But I am very sorry as I already deleted it. You can call KKM staff and ask them to send to you. It is only 1 piece. For this I really don’t know if you need to re-submit (for those who used the conventional way).

3 ways to submit Borang Maklumat Diri KKM with your documents (no need to verify).

-By hand to Putrajaya
Unit Pengurusan dan Profesional
Bahagian Sumber Manusia
Kementerian Kesihatan Malaysia
Putrajaya.

-By pos

*If you get the new KKM form, all addresses are written there.

*If you do not have to re-submit the KKM form, just submit MMC letter that you get by one of those methods mentioned above (but if by hand/ pos jangan bagi yang original lah.)

Once you settle your KKM and MMC form, chill and wait for SPA confirmation letter.

The eligibility to apply for eHO does not depend on how fast you submit your KKM/MMC form but it depends on SPA confirmation letter. After you pass your exam, your faculty will have a senate meeting. Then only they will send your names to SPA. Once SPA receives your names, they will send confirmation letter. If you get the letter before eHO application is opened, you are eligible to apply. Otherwise, sadly you have to wait for the next eHO (this was what happened to us. Sobs)

2nd batch of eHouseman

Salam.

Okay lari sikit from travel stuff.

Important alert for all new future housemen who just graduated this year (bukan from UM sahaja). many of us are very concerned as no information regarding this new system. we failed to get the 1st batch of eHO because we didn't get the confirmation letter from SPA.

so this is the full email i got from Mr Goh, ehouseman officer this morning.

this was sent directly to my email. bukan spam ok. 

Kalau malas baca. The main point:

-the next induction will be on 2ND WEEK of AUGUST
-eHO application will be opened 1 MONTH before that which is in JULY

Now you can plan your time. Be ready for future notice. Info from my UM batchmate saying that SPA is distributing our confirmation letters this week. Those who get the letters will be eligible to apply for the next eHO in July, insyaAllah.

Budget Itinerary Japan. The simple one. Part 1: Tokyo

Salam.

First of all let me clarify with you guys what kind of trip it was for us.

It was a budget-nature-chillax trip. so dont expect fancy things okay. ^^

I cannot give like very detail itinerary. I tend not to remember details of non-medical stuff since medical school. haha. so ni secara konklusinya.

p/s: 100 yen = ~RM3.10 (the rate i got masa exchange money)

Flight from KL - Haneda 26 April 2015. Sampai malam waktu Japan. So nak jimat just tidur airport. tapi i didnt sleep. sebab tak boleh tidur. i memang jenis susah nak tidur kat tempat baru. tapi wifi japan ni memang laju mcm bullet train dorg. habis berepisod-episode aku tengok drama. hehe.

So basically tidur Tokyo 3 malam aje.

Tokyo Day 1:

-Letak bag kat Hotel Ueno sebab awal sangat untuk check in. Airport to Ueno 610 yen.

-UENO PARK. Free. Jalan kaki je dr hotel. Patutnya bersakura sini. tapi dah habis. sobs.



-ASAKUSA: train return: 240 yen

Nakamise shopping. Beli air mineral 130 yen je. (zombie. dlm flight x tidur & kat airport pun tak tidur. so ingat ada mood nak tabur duit? O___o)


 Tokyo Skytree. Admission fee mahal 2060 yen. so tengok dari jauh jek.

Sumida River. Where we saw Tokyo Skytree dari jauh. Jalan dr Nakamise Street 10mins je.^^



Tempat yang tak pegi (or maybe aku tak ingat pegi) is Sensoji Temple. Anyway semua tempat di atas berdekatan je antara 1 sama lain. so jalan kaki je.

*okay travel partner baru je whatsapp bagitau we did go. cuma aku je ignorant tak nampak. hahaha. zombified sgt. Sensoji temple ni kena lalu Nakamise Street. It's at the end of the street.

- Ameyoko Market at Ueno.

tak ada gambar sebab i was in dreamland. kawan2 je yg pegi. aku tidur kat bilik setelah seharian menyombikan diri. Dorang kata banyak makanan murah esp sushi kat sini.

DAY 2

Mount Fuji.

personal review: a must place to go. ni je tempat kat tokyo (selain odaiba) aku rasa berbaloi tabur duit.

naik bus to Kawaguchiko dlm sejam lebih. Ticket return 3500yen. then we went to heaven of flowers at Moss Flower Festival. Admission fee + bus ticket return 1900yen.



kat sini jugaklah dapat jumpa sakura. puas gila hati che. alhamdulillah. tapi sedih sebab awan tebal so tutup mount fuji. patutnya boleh view dari sini tapi takde rezeki nak tengok. sobs.

demi nak amik gamba dengan mt fuji jugak so we jalan kaki around 10 mins dari kawaguchiko station to kawaguchiko lake. mula2 tak jumpa pun. then tanya2 orang katanya kena jalan lagi sampai hujung lake tu. demi mt fuji diteruskan perjalanan and taraaaaaaaaaaaaa



rasa nak menangis bila setelah jalan punyalah jauh jumpa jugak walaupun secoet tu je. and along the way jumpa sakura berimbun-rimbu. i felt like each cent of my money worth it. :')

it was a one-day trip. a worth one. so jangan rushing2 sumbat itinerary lain. enjoyyyyy. even otw balik dalam bus i couldnt take my eyes off mt fuji till the last sight of it that i could see. :')

DAY 3

-AKIHABARA

untuk peminat anime. of coz not me. mula2 masuklah jugak kedai but lama2 dok luar kedai je. at the end i decided nak buat solo tour. daripada bazir waktu dok luar kedai baik teruskan perjalanan. lagipun i dont want my travel partners rasa bersalah pulak bcz i tunggu dorang.

-TOKYO STATION

Popular sebab architecture-nya ala-ala omputih gitu.

-ODAIBA

Train ticket return 640 yen. mahal but berbaloi. banyak tempat nak explore tapi entah malas. bila dah sampai tepi pantai tu... dah tak rasa nak pegi mana-mana. i just stayed kat pantai tu menikmati keindahan Tokyo for almost 3 hours. :)





Ini list yang my travel partner bagi. apa nak cari kat Odaiba:
-Rainbow bridge
-Gundam statue
-Fuji tv building
-Liberty statue

No worries kat train station nanti ada pamplet yg lengkap map odaiba. amik. free je. i never studied about adoiba beforehand. redah je hari tu solo travel. dgn my sense of direction (puiiii) i made it. alhamdulillah. it was a great experience.

-TOKYO TOWER

I didn't go. but my travel partners did. nampak cantik bila night view. siang macam tiang elektik kat malaysia jek. hahaha. ada admission fee if nak naik. tak sure berapa.

DAY 4

Check-out dari hotel. Pegi Shinjuku station letak luggage. the biggest locker kat japan can fit 1 big luggage + 2 cabin luggage. gila besar. 600 yen for a locker.

-HARAJUKU


Takeshita Street

Kat sini ada Daiso besar. boleh beli souvenirs. if pandai cari boleh jumpa barang murah. bear in mind murah in japan as in 900 yen to 1000 yen. rarely jumpa price that goes lower than that. ada kedai 300 yen (mcm RM5 shop in Malaysia) tapi barang2 kat situ tak japanese. so tak layan. sini pun ada jual crepe sedap-sedap. 

-SHIBUYA

Jalan kaki je from Harajuku. 15-20 mins depends on your walking speed and sense of direction (puii). 

what to look for in Shibuya:
-Hachiko statue (dog & its owner. tak minat so tak cari pong)
-fascinating X crossing zebra. 

this photo was taken from 2nd floor Starbucks. ada video tapi berat sgt nak upload sini. when the light turns red, it's so fascinating to see the road suddenly fills with humans.
*From the picture above, nampak tak bangunan putih tu? 6th floor shopping mall tu ada nursery. untuk perempuan, you can chill and solat there. selesa sangat. ada air panas boleh buat maggi. ^^

-MASJID TOKYO @ Yoyogi Uehara

dari Shibuya, PLEASE take a train to masjid ni. menangis woo jalan kaki. from google map we found out that 8 min je dari shibuya. tapi jalan-jalan tak sampai2. and 1 hour later baru sampai. it's freaking far kalau jalan. terus dapat blister. boleh baca my kisah 'Aku Dia dan Masjid Tokyo' kat insta/fb. sadis. 

berbaloi bcz sgt cantik. takde kesempatan nak solat sebab masjid dah tutup. tapi ada Turkish guy yg jaga masjid ni sangat baik. bagi air, bagi makanan. 

Met a newly reverted sister, Fyurina. Welcome to the family. May you become a great Muslim. Amin.
Last stop terminal bus untuk berangkat ke Osaka. Night bus 8 hours journey 3800 yen berbanding 13k yen shinkansen. Night bus ni selesa & jimat hotel 1 malam. terlalu penat hari tu so tidur as soon as naik bus sampai pagi bukak je mata, OSAKA!!! 

*cerita sedih bus terminal ni juga boleh baca kat fb/insta. sobs

Tips: 

WIFI. Japan punya wifi network for foreigner ni maju sangat. available kat most of tourist attractions & train stations. sampai je airport, cari mesin to register which requires your passport (sebab tuk foreigner je). then nanti dia bagi no pin and download Travel Japan WIFI apps. masukkan no pin & there you go. Free wifi around Japan. btw tak perlulah nak on9 24/7. enjoy your holiday!

Terminal Bus. Lain bus depart dari terminal yang lain. so kindly check your ticket. jangan jadi macam kami lagi 15 min departure baru sedar salah terminal. dgn kaki blister berlari bersama luggage. not fun bro. time tu rasa nak nangis. tapi actually it was a kenangan yg takkan dilupakan. 

Tisu basah. Japan takde air dalam toilet. so nak beristinjak, senang kalau ada wet tissue. lagi bagus if ada botol kosong. but i carry around wet tissue. mudah dan selesa dan tak berat.

*Another place i didnt go, Ikebukuro. Peminat pokemon kena datang sini. kalau tak minat boleh skip macam i. hehe. 

*why tak pegi Disneyland? most of us love nature and besides that we were on tight-budget. kalau minat tu, berbaloi jugak nak tabur duit. tapi since tak minat & budget pun chokia je, lupakan.

thats all for my trip in Tokyo. Simple. Chillax. Budget. No rushing. 1st day tu aku tidur je seharian. usually malam tak stay out sampai midnight. by 8 or 9 dah ada kat bilik. for me, travelling is not a race that you must rush to go to all places. take your time & enjoyyy. it's okay to do nothing when travelling. after all it's a holiday.

Overview Budget Japan

Salam.

I have lots of time but let's get straight to the point. Tulis panjang-panjang nanti orang cakap cliche. haha. Post banyak salah, tulis banyak salah and tak post apa-apa pun salah. Lumrah manusia. No matter what you do, ada aje yang tak puas hati. pfffff. you cant please everyone. so just live the way you believe okay ( yang penting tak bertentangan agama dah lah. ^^)

To travel, 1st of all you must recognize what kind of trip you want it to be: luxury? Backpacking? Eduational? Chillax?

Determine your budget lah dulu. My parents tak kaya sesenang nak tabur duit so my trip is always a budget-trip. Kalau nak luxury trip, tak payah susah baca semua ni pegi aje cari agen ok? ;)

Dah ada bajet then baru fikir bajet tu agak-agak sesuai dengan destinasi mana. Kalau bajet RM2k - RM3k takyahlah berangan nak p Europe tour. Duit ticket flight pun belum tentu lepas.

My hobby is looking for cheap flight tickets. Hari-hari bukak skyscanner (boleh cari apps ni) check harga tiket. tapi kadang-kadang harga kat apps ni tak sama dengan the real price. tu hanya anggaran. so bila dah detect tiket mana murah, baru bukak that flight company website to view the exact price. like i mentioned, mak ayah tak mampu nak tabur duit so tak mampu nak naik even MAS (how ironic a Malaysian cannot afford her own country national flight. sobs). jadi selalu memang terus check airasia. biarlah orang kata tak glamour pun. tapi dengan duit sendiri as a student dah merata aku fly dengan AA.

Last October dah pegi negara omputih so cuti kali ni nak bercuti nagara asia pulak. nak pegi Europe trip tak kesampaian so i eventually chose Japan.

My total belanja for Japan 10 days trip ~ RM2300

sorry tak dapat nak bagi exact figure sebab my travel book hilang. sobs.

RM2300 untuk:
-flight KL - Haneda Tokyo
-flight Osaka - Seoul Korea
-bus Tokyo - Osaka
-penginapan Tokyo
-penginapan Osaka
-admission fees
-train fees
-makan
-souvenirs
-pembaziran (LOL)

percaya tak? HAHAHA. macam tipu tapi betul itulah hakikatnya. kenalah percaya sebab dah banyak kali cakap my parents bukan yg boleh tabur duit.

Harga flight ikut rezeki jugak. kena tau apa harga murah yg AA boleh bagi. Return Japan ticket AA boleh dapat as low as RM550. so kalau dah nampak one-way ticket pun dah RM700 takyahlah beli. tu mmg mahal,

This was what we got:

KL - Haneda = RM399

*Haneda Airport lagi dekat dengan city dari Narita Airport. KL - Narita masa tu dapat RM369 tapi bila kira airport transfer, lebih kurang aje. so we chose Haneda.

Tokyo - Osaka naik night bus dapat 3800 yen. Dalam ~RM110 gitu. Perjalanan 8 jam. If nak cepat and jenis yang mampu tabur duit, naiklah Japan Bullet Train @ Shinkasen 1-way ticket RM400+ 2 hours plus je journey.

Osaka - Seoul ~ RM340 flight Peach (Japan low cost flight company). Tu kira mahal lah sbb time we nak pegi tu is Golden Week kat japan. so harga tiket mahal. Harga Osaka - Seoul naik Peach ni can be as low as RM100. tapi kalau aku dah mampu tabur duit sikit, tak naik dah kot. kapal terbang selesa je. tapi system luggage dorg tak munasabah. beli 40kg worth baggage tapi boleh masuk 2 luggage je. even tak cukup 40kg pun tak boleh masuk luggage lain. haih. pastu hand-luggage memang dorg timbang must be less than 10kg. lebih seKg pun tak boleh. tak boleh nak tipu sebab before immigration pun dorang tunggu tuk timbang berat bag. double sigh.

Mode of transportation around japan - train and jalan kaki. bagi yg fit, selagi boleh jalan kaki jalanlah sebab transport dorang terlalu mahal. Drp RM2300, RM500 train saje.

*why tak amik 7-days JR pass for foreigner? sebab untuk kami tak berbaloi. kalau kira train yg kami guna + bus tokyo ke osaka baru dlm RM600. Tapi harga JR pass tu almost RM1000. JR pass ni tak boleh pakai for other private railways. so tak berbaloi untuk itinerary kami.

admission fee - to be honest sepanjang kat japan kami pegi tempat yg mostly free je. haha. masa kat tokyo yg we spent banyak for Mt Fuji je. yang lain free. Mt fuji tu pun dah termasuk dlm transport fee. so basically tokyo we paid nothing.

in Osaka/ kyoto admission fee tuk masuk kampung jepun pakai kimono tu. admission 300 yen student price (berguna jugak card matric um yg dah expired tu. hehe) and nak pakai kimono 200 yen. so total dlm RM16. tu je admission fee sepanjang kat Osaka/ Kyoto. haha. baru perasan betapa bajetnya kami. LOL

Accommodation

Tokyo we stayed in Ueno Hotel. kteorg save kat sini. Paid hotel ni harga for 2 orang but we sumbat 5 orang. 4 hari 3 malam sorang bayar RM167.

*hotel senang nak sumbat orang but if guesthouse/ hostel susah sebab memang kira ikut kepala & tempat kecik so tak boleh nak sorok2 seludup orang. that is why i chose hotel instead of hostel / guesthouse untuk trip ni. :)

We prepared for this trip seminggu je. Since ada experience travel negara2 maju so i dont need masa yg banyak nak study. masa 1st trip dulu study 3 bulan baru travel. now dah pro sikit. eceh. most of developed country their transport system almost sama. so tak gaduh. and my sense of direction kinda good (big fat LOL). since dah travel ni agak bijak la baca map. mcm masa dorg g Akihabara & i did my solo travel to Odaiba, Tokyo station. tak study langsung pasal tokyo. alhamdulillah dgn skill baca map jalan & map subway selamat semuanya. HAHAHA. lagipun i had 4 other brains to think the itinerary. so takyah nak pening kepala. masalah skarang we taktau yg tarikh we nak pegi ni Goldon Week in Japan. sobs. so ALL accommodation yg murah dah sold out. yang tinggal hanya hotel yg 1 malam RM8000. *pengsan*

dengan izin Tuhan, 1 of my travel partners ni ada kawan kat Osaka. maka kawan yg baik hati telah bersetuju tuk bagi kami tumpang rumahnya. weeeeeeeee! alhamdulillah. so save bajet. TAPI jangan happy sgt. sebab tinggal kat Osaka so we have to travel ulang alik ke Kyoto. dan harga transport to Kyoto dr Osaka almost sama dah dgn sewa hotel. haih. tapi still banyak jugak save. we stayed 5 hari 4 malam. selesa sangat, :)

Makan

Ini hal penting. Bukan remeh. Tak makan boleh jadi gaduh wooo. Travel negara sejuk, banyak jalan so cepat lapar. but you can save A LOT kat sini if bawak makanan dari tanah air.



HEHEHE. Okay tu bukan aku sorang punya. 1 bag tu share 2 orang. berbaloi sangat. kenyang, tak habis duit and less homesick. Masa 1st trip aku ke Korea last year, makan roti je 8 hari okay. sadis gila. and by hari ke 5 macam tu dah homesick bagai nak gila. sampai malaysia melahap nasi lemak, ok cerita korea later.

Beli Abam Brahim. ada 2 jenis. satu siap dengan nasi like nasi goreng & etc. satu lagi lauk aje like kari ayam, rendang daging. i bought both types. yg lauk tu beli nasi impit & rebus aje.

tapi i still beli food kat sana like takoyaki, onigiri & etc la. after all if travel kena jugak rasa their food kan. kalau tak, tak jepun lah kita, LOL.

My makanan 1 luggage tu worth RM147 tuk 15 hari. beli kat giant je. and my mom masak sambal goreng ikan bilis. tahan 2 minggu. ^^

Souvenirs dan pembaziran perlu ke nak cerita?

okay i give overview lah. tshirt i love japan bagai paling murah 900 yen. tak jumpa pun yg kurang dari tu. dia tak jual 3 RM10 camtu. mcm korea boleh dpt 3 baju 10k won. Japan nope. yes mahal. so i just bought tuk my father & adik. adik-bradik lain dah kawin. if beli untuk dorg kena beli untuk ipar2 jugak. so i decided beli tuk anak mereka sahaja. titik.

most of my souvenirs beli kat Daiso kat Harajuku. hehe. okay Daiso sini lagi murah dari malaysia. 1 barang 100 yen (~RM3). yang penting dari Japan. titik.

Greentea! it's a must. beli lah apa pun bentuk makanan from greentea. im so obsessed. syurga greentea in japan. kitkat greentea, biskut greentea, aiskrim greentea, doriyaki greentea & etc. menyesal beli sikit je. sobs.

okay tak shopping sakan pun kat Japan sebab mahal.

Duit poket yang bawak initially 30k yen je (~RM900) tapi sampai Japan tengok mahal-mahal belaka so keluar lagi 10k yen. tapi ekceli tak habis pun guna. Kalau tak buat pembaziran tak perlu pun 10k yen tu. haha. dengan pembaziran yg berlaku, lebih just 5k yen yang dibawa ke Seoul dan ditukar. ^^

Tips travel:

-if tak fit, tak payahlah bawak backpack gedabak tu. walaupun tak dapat title backpacker (so what?) tuk kemudahan anda, bawak aje luggage beroda. and make sure RODA 4 bukan 2. travel ni banyak guna tenaga. so simpan la tenaga. kalau 4 roda tolak je luggage tu takyah tarik. ^^

-bawaklah buku/ download ebook supaya anda tak meninggal dunia kebosanan dlm flight & subway but jgn la bawak novel yg tebal hardcover pulak tuh. Mitch Albom punya buku ringan je. content pun bagus.

-jangan share anything dengan travel partner unless memang family members. sebab bila travel ni penat, semua jadi berkira. share luggage tp sorang aje yg bawak. yg penge-share pun buat2 tak paham bahasa. spoil mood bro!

-bawak makanan dr malaysia. takyah membazir kat food hari2. boleh je makan but takyahlah hari2. you can save a lot.

-study la before travel. unless you have sense of direction mcm gua. #puiiii

-bawak raincoat yg nipis & boleh lipat2 jadi kecik. or beli je dr watson/ guardian. selain tuk hujan boleh jugak alas for solat.

-ni keistimewaan perempuan, nursery negara maju ni maju sangat dan selesa. so guna nursery tu tuk solat, isi air panas & rehat.

-pakai kasut yg selesa & tapak tak haus. kalau tak blister lah jawabnya. dah ada blister menangis wooo.

-sampai airport pegi cari hardcopy subway map & town map. susah guna aplikasi semua ni.

-before fly call bank make sure card atm dah activate. wlp dah buat online (like me) just call. disaat perlukan duit atm card takleh guna. sobs.

-bawak sikit la cash usd/ myr. just for emergency.

-bila travel you are a team. so janganlah ada yg dipinggirkan. no such thing like pair pair bagai. orang yg dipinggirkan tu kesian, spoil mood. beli apa2 cepat2 nak pair dengan orang tu ni. then yang takde pair kena bayar sorang. tak cool bruh!

-siap2 order baggage for flight. supaya tak kecoh kat immigration. ada airport strict dan ada yg chill je. tapi tuk kesenangan, order je.

-pre-order food dlm flight. murah. bila para2 mugara/mugari keluarkan makanan, bau semerbak kot. if tak pre-order, harga makanan mahal.

-bila travel jgn lah bawak baju hitam berkabung je & tudung malas. bawaklah yg colorful. tuk perempuan, baju takyah bawak banyak tapi tudung bawak banyak yg colorful. then tukar2 je. ini untuk gambar yg cantik. :)

-monopod itu penting. dan if solo travel tripod paling penting. gambar is one of main event kan dlm setiap trip? so be prepared!

-bawak pen & buku nota tuk catat perbelanjaan. takpun save dlm phone. pen pun penting tuk tulis card declaration.

-janganlah waste ATP selagi mampu. kalau dlm train tu ada tempat duduk, just duduk. banyak lagi gunanya ATP tu. so use it wisely. ^^
